Yes, all patients pay at the time of service; however we will provide you with a preprinted insurance claim form to submit to insurance for possible reimbursement.
If your secondary insurance company will accept a claim directly from you, without going through Medicare, we can give you a secondary insurance claim form. Prior to receiving the secondary claim form, a consent must be signed stating that you will not submit your claim to Medicare.
You may be eligible to use your account towards services or supplements provided by Preventive Medicine. We encourage patients with these accounts to contact their plan administrator to find out what expenses qualify.
Whether you are reimbursed is dependent upon your plan benefits. We encourage you to call your insurance company and inform them that you are interested in seeing Dr. Rathod. The insurance company will be able to pull up Dr. Rathod and your plan benefits to see what kind of coverage you are eligible for.
This may happen from time to time. If the insurance company sends us a check we will do one of two things:
1.) We will sign the check over and mail it to you.
2.) If they took taxes out or if there are multiple patients on one check, we will send the check back to the insurance company and instruct them to dispurse the funds directly to you.
We recommend to have a primary care doctor, who accepts your insurance, so they can see you for routine issues as well as your annual physicals. By doing this, you are able to have your yearly exam submitted to the insurance company. We encourage you to use Dr. Rathod more as a holistic wellness consultant. If you choose, Dr. Rathod, can also be your primary care physician.
